## What is a Thyroid Nodule?

A thyroid nodule is an abnormal lump or growth within the thyroid gland, located in the front of the neck. Most thyroid nodules are benign (non-cancerous), but approximately 5–10% may be malignant, making timely evaluation essential.

## Signs & Symptoms

Many thyroid nodules do not cause symptoms and are discovered incidentally during a routine examination or imaging.

### Common Symptoms

* Painless swelling or lump in the front of the neck
* Visible neck swelling
* Difficulty swallowing (dysphagia)
* Sensation of something stuck in the throat
* Difficulty breathing, especially when lying down
* Hoarseness or change in voice
* Neck discomfort or pain (less common)
* Persistent cough not related to infection

## Diagnosis

### 1. Clinical Examination

* Detailed medical history
* Neck examination
* Assessment of lymph nodes
* Voice assessment

### 2. Blood Tests

* Thyroid-Stimulating Hormone (TSH)
* Free T3 and Free T4
* Serum Calcitonin (when indicated)
* Thyroglobulin (selected cases)

### 3. Ultrasound of the Neck (Most Important Test)

Ultrasound helps assess:

* Nodule size
* Solid vs. cystic nature
* Microcalcifications
* Irregular margins
* Increased blood flow
* Cervical lymph nodes

### 4. Fine Needle Aspiration Cytology (FNAC)

Recommended for suspicious nodules (usually β‰₯1 cm with suspicious ultrasound features). FNAC is the gold standard for distinguishing benign from malignant nodules.

### 5. Thyroid Scan

Performed when TSH is low to determine whether the nodule is:

* Hot (functioning)
* Warm
* Cold (non-functioning)

### 6. CT Scan or MRI

Used for:

* Large goiters
* Retrosternal extension
* Airway compression
* Surgical planning

### 7. Laryngoscopy

Performed if there is hoarseness or suspicion of recurrent laryngeal nerve involvement.
